File a complaint against one of the entities or individuals that the Iowa Insurance Division regulates.

Determine Eligibility

This form is for people who want to file:

  • An insurance complaint.
  • A securities or investment complaint.
  • A complaint about another regulated industry, such as a residential or motor vehicle service contract provider, a perpetual care cemetery, or a preneed seller.

You can file a complaint with Iowa:

  • Only if your policy was issued in the state of Iowa.

  • Only if you are covered by a group policy issued to an employer based in Iowa.

  • If the offer, sale, or purchase of a security or investment occurred in the state of Iowa.

You cannot file a complaint with Iowa:

  • If your policy was issued or purchased in another state.

Please contact the appropriate regulatory agency or insurance department of the state in which the policy was issued or purchased for help. Even though an insurance company has its home base in the state of Iowa, the state in which the policy was issued or purchased has jurisdiction.
 

Prepare Supporting Documents

Supporting documents are important.

  • Attach a photocopy of all documents, including letters, contracts, bills, checks (both sides) and other papers related to your complaint.
  • Submit supporting documentation by attaching the documents electronically at the end of your consumer complaint online or submit supporting documents by email, U.S. mail or fax.
  • Do not send originals. We may not be able to return them.

Mark the “Other” box under the “Type of Insurance” if the complaint is not about insurance, for example: securities, sale of cemetery merchandise, funeral merchandise or funeral services, residential service contracts, motor vehicle service contracts, retirement facilities, external review, etc.
 

How to File

After You File an Insurance Complaint

  • The Division will send a copy of your complaint to the insurance company and ask them to review and respond to us regarding your concerns.
  • After receipt of the company's response, we will review and ask for more information if needed and work with the company to address resolvable issues.
  • Typically, the complaint process can take up to 30-45 days from receipt of the complaint and our review of the information from the company.

Get Investigation Results from Insurance Complaints

  • We will provide you with an explanation of the company's response and our review.
  • We may suggest steps that you can take to have your concerns evaluated if it is outside of our authority.
  • Your complaint becomes part of the company's and/or insurance producer's record with our agency.
  • We monitor companies and insurance producers for trends in their conduct.
